2009 Data Finalization Deadlines and Instructions.

Dear CPQCC Participants:

This is a reminder that we are strictly re-inforcing the deadlines this year for both April 1st, June 1st, and June 7th deadlines.  To receive a "Superstar Center of the Day" award, please submit your April 1st requirements by Monday, March 22nd at 8:00am.

A confirmation e-mail is required from each Center's CPQCC Data Contact for receipt of the 2009 Data Finalization Guidelines (473.4 KB ) released on February 1st. Please review the guidelines for the process of confirming the CCS Reports. (For your convenience, we have also created a one-page 2009 Data Finalization Reference (85 KB ).)

Please also make sure that you submit the following no later than April 1st:

• Submission of CPeTS items C.1 through C.33 and Admission/Discharge items 1 through 52 for all infants born in 2009; Updated data for all Still In-House infants born in 2008
• The CCS Supplemental Form must be submitted.
• Review CCS Draft Report.
• CPQCC/VON Contact Report.
• Center Contact Change Form (169 KB ) (Only if you are updating the following: Hospital Name, Report Contact and/or VON Web Services Administrator).
• VON Membership Survey.
• Eligibility Verification Plan (16.2 KB ).
• ID Verification Form.

Please also make sure that you submit the following no later than June1st: 

• Confirm the Data Consistency Report - Duplicates and Re-encounters (Scheduled Release:  March 15, 2009).
• Review the on-line Confirmed Unknown Report.
• All records for 2009 and prior years are complete and 100% error-free.
• The Final CCS Report must be confirmed for accuracy and completeness.

Please mark your calendars for the six-day window period between June 2nd at 7:00 am until June 7th at 11:59 pm when we enable the Confirmation Button in the CCS Report:

• Confirmation of your Final CCS Report for completeness and accuracy AND submission to CCS.  You can also access the Guide to Interpretation (202.3 KB ) on-line.

CCS Reports: If you need to update your report, changes will need to be made in either your CCS Supplemental form or your CPQCC data. AFTER you have made these changes, please make sure to contact us at support@cpqcc.org to rerun your report. Please acknowledge that your updates will NOT be reflected in your CCS report without a rerun. (Note: This applies to both preliminary AND final CCS reports).

Please note that if we do not receive ALL of the above by the designated deadlines, your Center will NOT be receive any of the annual reports from CPQCC, CCS or VON.
