Dear CPQCC Participants:
We are pleased to present a new contact list, known as the "CPQCC & HRIF QCI Directory" which will replace the Data Contact Roster. This revised directory will include CPQCC member contacts and High Risk Infant Follow-Up (HRIF) Quality of Care Initiative (QCI) HRIF Programs contact information. It will be sorted by hospital name and the password is the same as it was for the previous Data Contact Roster.
Currently, we have included contacts for 129 CPQCC Hospitals and 64 HRIF Programs within the state of California. CPQCC member hospitals that do not have a HRIF Program onsite, a "No" will be displayed and the hospital providing follow-up care will be listed, under "Hospital Providing HRIF Program Services".
NOTE: The information included in this directory consists only of what CPQCC & HRIF QCI currently has on file. Please contact us immediately if any of the information is inaccurate or needs to be updated. Please print out your hospital form, update the information and faxing it to us at 650-721-5751.
The purpose of this directory is to motivate communication between the NICU and HRIF Programs. Please read the excerpt below from the HRIF-QCI website:
"CCS Standards for NICUs require that each CCS-approved NICU shall ensure the follow-up of high-risk infants discharged from the NICU. Each NICU shall either have an organized HRIF Program or a written agreement for the provision of these services by another CCS-approved NICU."
