2007 Data Finalization Reports
Dear CPQCC Participants,
Your Center's CPQCC Finalization Report is currently available to view. Please acknowledge that these reports were created on 6/03/08. However, if you have recently submitted any finalization documents, please contact us directly for an updated report at support@cpqcc.org.
Please click on the zipped file link below and follow the instructions.
2007 CPQCC Data Finalization Reports
- Find your Center's name in the list and then click the PDF file to view and save your report.
- Use your data password to open then link.
- Submit all (if any) "pending" documents via fax to 510-620-3144.
- Make all (if any) corrections to documents that are "incomplete".
- Make all (if any) corrections to your online CCS Report.
Please note that due to the file size, it may take a few minutes for the zip file to load. If you are unable to download zip files please contact your center's Local Network Administrator or Helpdesk.
The grey box at the bottom of your Finalization Report is to be filled out by each member. Please only fax in if your Center has SIH babies and indicate the amount and the corresponding IDs. Please leave the status section blank.
We want to thank all of you in your hard work and special efforts to submit your 2007 data. As a reminder, the www.cpqccdata.org site has been locked out which means you cannot ADD any new 2007 data. However you may still EDIT any existing 2007 data that is currently in the database. To request an unlock AFTER closeout to enter 2007 data please send an email to support@cpqcc.org.
