2009 Contact Report and Membership Survey
By April 1st, please submit the following 2009 finalization documents:
- CPQCC/VON Contact Report.
- Center Contact Change Form (Only if you are updating the following: Hospital Name, Report Contact and/or VON Web Services Administrator).
- VON Membership Survey.
- Eligibility Verification Plan.
- ID Verification Form.
A confirmation receipt of the 2009 Data Finalization Guidelines released on February 1st is required from each Center's CPQCC Data Contact in order to receive a copy of the Center-specific CPQCC/VON Contact Report and the VON Membership Survey.
With the confirmation receipt, the Data Center will provide instructions on how to access the Center-specific reports:
- Click on the zipped file link below and follow the instructions. 2009 Finalization Documents_VON Surveys and Contact Reports.zip (32.1 MB) (note: Your CPQCC/VON Contact Report is attached to the VON Surveys as a PDF file).
- Find your Center's name in the list and then click the PDF file to view and save your CPQCC/VON Contact Report and VON Survey.
- Use your cpqccdata password to open the link.
- CPQCC and VON require each Center to indicate who will serve as the Team Leader, as well as, confirm the contact information for each Center's designated Report Contact, Data Contact(s), Administrative Contact, Neonatologist, Quality Improvement Contact(s), Transport Contact(s) and CEO.
Please allow a few minutes for the file to load as it has documents included for all Centers.
