Dear CPQCC Participants,

A few changes have been made to the April 1^st Data Finalization deliverables for the 2010 close out. 1) We will no longer collect the Eligibility Verification Plan. We now have other resources where we can retrieve this information. 2) The VON Membership Survey will now be submitted on-line via the VON Member Web Services website. If your center does NOT have access then you should have received an e-mail on November 15^th with instructions on how to access this site and the VON Membership Survey. 3) You will notice that your CPQCC/VON Contact Report has been slightly changed in that you will only need to sign and date page 3. We have deleted all duplicate persons who were in both categories of Contact #1 and Contact #2 (i.e. Data Contact #1 & #2; Transport Contact #1 & #2), we ask that you record two different individuals in these categories. The purpose of this is to capture only those hospitals that have more than one person to fill these positions. If there is only one, please record that person once as Contact #1 and leave Contact #2 blank. Also, please make sure to check all e-mail addresses for accuracy.

We wanted to re-iterate the purpose of the Administrative Contact person. The Administrative Contact is "Responsible for Financial and Membership Agreement information", which is listed next to this title on the CPQCC/VON Contact Reports. This includes all related CPQCC contract issues as well as the annual membership invoices. We've recently encountered that some of the current Administrative Contact persons do not handle one or more of these responsibilities. For this reason, we ask that you review this role with your team and make the necessary adjustments if needed.

Data Finalization Schedule

February 1^st:

An e-mail will be sent to each Center's CPQCC Data Contact with information regarding 2010 Data Finalization. A confirmation e-mail is required for receipt of the e-mail and the link to download the 2010 Data Finalization Guidelines which will be released on February 1^st. Please review the guidelines for the process of finalizing your data and confirming the CCS Reports. You will notice a zipped file in the above deliverables for the CPQCC/VON Contact Reports. Please follow the instructions below to retrieve your center's report.

Please use the same password as used to open the CPQCC & HRIF-QCI Directory to open the file (this will be the same password as old Data Contact Roster).
Find your Center's name in the list and then click the PDF file to view and save your CPQCC/VON Contact Report.
CPQCC and VON require each Center to indicate who will serve as the Team Leader, as well as, confirm the contact information for each Center's designated Report Contact, Data Contact(s), Administrative Contact, Neonatologist, Quality Improvement Contact(s), Transport Contact(s) and CEO.
Make the necessary changes on each of the three pages. Then sign, date and fax it to the CPQCC Data Center at 650-721-5751.

February 8^th:

Confirm the electronic receipt of the Data Finalization Guidelines by responding to the email you received.

March 18^th:

Would your center like to qualify for the "Superstar Center of the Day" award? If so, please submit ALL of the April 1st deliverables by March 18^th this year. If your Center qualifies, it will be listed in the link below at that time.

Please note that we are once again strictly enforcing the deadlines for the April 1^st, June 1^st and the June 7^th deadlines this year.

April 1^st:

CPeTS Items C.1 - C.33 and the Admission/Discharge items 1-52 for all infants born in 2010; Updated data for all Still-In-House infants born in 2009.

CPQCC/VON Contact Report.
Center Contact Change Form (Only if you are updating the following: Hospital Name or Report Contact information in the above document.

June 1^st:

Confirm the Data Consistency Report - Duplicates and Re-encounters (Scheduled Release: April 11, 2010).
Review the on-line Confirmed Unknown Report.
All records for 2010 and prior years are complete and 100% error-free.
The Final CCS Report must be confirmed for accuracy and completeness. (Scheduled Release: March 15, 2010).
CCS Reports: If you need to update your report, changes will need to be made in either your CCS Supplemental form or your CPQCC data. AFTER you have made these changes, please make sure to contact us at support@cpqcc.org to rerun your report. Please acknowledge that your updates will NOT be reflected in your CCS report without a rerun.

We encourage you to analyze ALL of your CPQCC, CPeTS and CCS data to allow time for changes and updates to be made prior to June 1^st. This would include your finalization documents which are listed above.

Please FAX ALL printed 2010 Finalization Documents to the CPQCC Data Center (650) 721-5751.

June 2^nd through June 7^th:

Confirmation of your Final CCS Report for completeness, accuracy AND submission to CCS. You can also access the 2010 CCS Report: Guide to Interpretation by clicking on the link below.

Please note that if we do not receive ALL of the above by the designated deadlines, your Center will NOT be receiving any of the annual reports from CPQCC, CCS or VON.

June 28^th:

Your Center's "2010 CPQCC Data Finalization Final Status Reports" are currently available in the link at the top of the page. Please confirm that your report was received by June 28^th by submitting a "received" response to support@cpqcc.org. This action is listed as number 13 in section III of your Data Finalization Guidelines.

At that time you can click the zipped file link above in the list of deliverables and follow the instructions below.

Find your Center's name in the list and then click the PDF file to view and save your report.
Use your CPQCC data password to open the file.

PC: After clicking on the link, a winzip window should pop up. Click open, wait a minute or two and then choose your hospital in the list.

MAC: After clicking on the link, the zip file will automatically save to your downloads area. Find the file, unzip (using an unzip app, i.e. Stuffit Expander) and find your hospital in the list.

Please note that due to the file size, it may take a few minutes for the zip files to load. If you are unable to download zip files please contact your center's Local Network Administrator or Helpdesk.

We want to thank all of you in your hard work and special efforts to submit your 2010 data. As a reminder, the www.cpqccdata.org site will be locked out which means you cannot ADD any new 2010 IDs. However you may still EDIT any existing 2010 data that is currently in the database.
