How does an infant get enrolled in an HRIF Program?

All CCS-approved NICUs are required to either have their own HRIF clinic or be able to refer infants to an external HRIF clinic. As part of the discharge process, the NICU will identify any infants that are potentially eligible for HRIF and refer them to the appropriate HRIF clinic. The HRIF clinic will receive notification from the referring NICU or the local CCS County Program/Regional Office that the infant or child has been authorized to receive follow-up care services. Note: Entering infants into the CPQCC’s NICU and HRIF Databases does not refer them for HRIF services; these databases are for tracking purposes only.
